Holy Family Supplementary Admission Forms
As Holy Family RC & CE College is a Voluntary Aided school we are our own admission authority. As such parents are required to complete both the general high school application to Rochdale Council AND the school’s supplementary forms.
Please ensure you read the school’s admission policy which will contain all the information about the oversubscription criteria.
There are two forms to complete:
HFCH Form A – must be completed online using the link below. (Do not complete until you have read all the information below).
HFCH Form B – this must be printed and completed by the clergy at the church you attend if you are claiming priority based on church attendance.
Please ensure you read the User Guide which contains step by step instructions on completing the Supplementary Forms.
The application for Rochdale Council AND the submission of the School’s Supplementary Forms must be completed by 31st October 2024
If you have any issues completing the forms further information will be given at Open Evening (Thursday 3rd October 2024) and following this there will be parental drop in sessions for assistance. Frequently Asked Questions
My child is not baptised, do I still need to complete the supplementary forms?
The forms are used to provide evidence of Baptism and church attendance. If your child is not baptised it is still helpful to complete the supplementary forms so we have your details which may include details of siblings or feeder schools which may also give priority to your application. Instead of uploading the Baptism certificate to your HFCH Form A please upload a statement that confirms your child is not baptised.
My child already has a sibling attending the school, do I still need to complete the Supplementary Forms.
Yes, Supplementary Forms should be completed for all children who are claiming priority for a school place based on grounds of faith and / or church attendance.
I am unable to print the Form B for my local church to complete?
Please contact the school directly on 01706 360607 and we will be happy to provide a paper copy of Form B for completion.
I do not have computer access and am unable to complete Form A online?
The Form A must be completed online, however, following the Open Evening on 4th October 2024 there will be parental drop in sessions to provide support to parents to complete online.
My child has been baptised / christened in a different faith, do I still need to send copy of the Baptism certificate.
Yes it is still worth completing the Form A and sending the evidence as this may give additional priority under category 2 of the admission policy.
The church I attend isn’t listed in the admissions policy, do I still need to complete Part B?
Yes it is still worth completing the Form A and submitting the competed Part B form from your church, as this may give additional priority under category 2 of the admission policy.
My child is Baptised but I do not attend church so can not get a Part B completed.
Complete Form A and upload evidence of baptism, however in place of Part B please upload a statement confirming that you do not attend church. The Form A can not be submitted without a document uploaded in each section.
I have submitted the forms but have not received an email confirmation with a copy of the forms.
If you have not received an email confirmation with a copy of your forms then your application has not been received by the school. Please resubmit your application or contact the school for further assistance.
